3 years old, SPEECH and ECSE

Polio - 3 doses

DTaP/DT/DTP - 4 doses

Measles, Mumps & Rubella - 1 dose on or after the first birthday

Hepatitis B -3 doses

Hepatitis A - 2 doses - first dose after the first birthday

Varicella - 2 doses - after the first birthday

Pneumococcal - 2 doses after the first birthday

Haemophilus Influenzae type b (HIB) - 3 doses

4 years old, Pre K, SPEECH, ECSE

Polio - 4 doses, including one dose after 4th birthday

DTaP/DT/DTP - 4 doses, including one dose on or after the fourth birthday

Measles Mumps Rubella (MMR)- 2 doses, 1st dose on or after the first birthday, 2nd dose after the fourth birthday

Hepatitis B -3 doses

Hepatitis A - 2 doses after the first birthday

Varicella - 2 doses after the first birthday

Pneumococcal - 2 doses after the first birthday
